When the owner or licensed contractor sends an agent or employeer to sign for the license or permit, a notarized Letter of Authorization (PDF) is required.
The above PDF documents require Adobe Reader to view. The forms may be filled out on-line, printed and mailed to the Business License Division at the address listed below.
Applications are also available in person from the Business License Division, City Hall, Bloomfield Avenue at 183rd Street, Cerritos, CA 90703. Applications can be requested by calling (562) 916-1236 or by submitting an e-mail to Business License and asking for a packet to be mailed to you.

Applications sent via fax will not be accepted.
All contractors who hire sub-contractors in connection with a building permit must complete and submit the Sub-Contractor's List to the Business License Division three days prior to the request for final inspection. All sub-contractors must obtain a City business license before the final inspection takes place.
Please complete all fields including the date on which work was completed for each sub-contractor. If the general contractor also performed the specific field listed, then fill out the general contractor's name on the form in that field.
